For clinicians and prescribers who are prescribing for veteran patients, we are licensed under FSS contract #36F79725D0018. 

1. Clinician writes the order/consult for the Veteran and sends the order to Prosthetics
2. Prosthetics creates the Purchase Order and sends a fax to IME
3. IME processes the order the same day
4. Order ships directly to Veteran or to the facility depending on request
5. IME finalizes the order with the credit card process

If you are prescribing for a veteran patient who is not placing an order through their local VA health center, providers must fill out VA Form 10-10172 (Request for Services). 

General Instructions:

1. Clinician fills out DME Request section of the VA RFS Form 10-10172 (Page 2 Only)
2. Fax or Secure-Email the form, along with any office notes and plan of care to the local VA Community Care office. You can find a list by state (states on the right-side) listed here: https://department.va.gov/vha/community-care/care-coordination/facility-contacts/
3. The Form will be given to a reviewer at the VA, then passed to prosthetics to place the order with Innovative Medical Equipment, LLC.
